City of Williams
Investigates municipal finances, procurement, executive oversight, staff treatment and transparency. The jury issues 11 findings and six recommendations covering governance/ethics training, credit-card controls, executive hiring, staffing review and public completion reporting. Allegations and Brown Act concerns are attributed to the jury, not independent legal findings. The Council disputes the findings; Councilmember Troughton supplies a distinct response. This unchanged 23-page annual report contains the investigation on physical PDF pages 8–13 (printed pages 1–6).
2025–2026 report
Inspection of the Colusa County Jail
Examines the existing linear jail, new podular facility and planned transition of inmate medical care from WellPath to the County. Four findings describe professional operation, inspections, design risks and the medical transition; R1 calls for close monitoring of County-provided care. The Board response is supplied, but no separate Sheriff response is in this intake. This unchanged annual report contains the investigation on physical PDF pages 14–16 (printed pages 7–9).
2025–2026 report
Colusa County Civil Grand Jury Special Education Investigation Report
Examines IEP service delivery, staff training, parent communication, student safety, placement capacity, assistive technology and dignity. Eight findings and eight recommendations call for record integrity, tracking/remedies, training, oversight, reporting and service capacity. The joint CCOE/SELPA response disputes systemic characterizations and describes planned changes. Its F2/F3/F4/F6 paraphrases differ materially from the published findings; those nominally numbered audit links are explicitly unscorable. This unchanged annual report contains the investigation and glossary on physical PDF pages 17–23 (printed pages 10–16).
