That the information recorded by Animal Control includes (if available): F1, F2, F3, F4, F5, F6, F7 ● the gender and age of both the victim and the dog or dogs, ● a photograph accompanying notation of the suspected breed of the dog (if possible), ● whether the dog was spayed/neutered, ● the dog’s vaccination status, ● location of the attack, ● extent of injury, ● the treatment necessitated, ● any prior history of aggression by the dog, ● whether the dog was quarantined, and ● legal outcome. (e.g., fines, lawsuit, euthanasia) (F-7)
